Introduction

Cervical cancer is one of the most serious health problems affecting women worldwide, and effective prevention efforts through early detection are crucial in reducing its risk. One method used for early detection of cervical cancer is the IVA (visual inspection of acetic acid) method, which involves a visual cervical examination using acetic acid to detect abnormalities. The early detection program for cervical cancer with the IVA method has been implemented in all Puskesmas in Batam City, including the Sei Lekop Puskesmas, since 2015. However, despite the targets set to achieve a 50% participation rate among women of childbearing age (WUS), the actual participation rate remains low, at only 1.78% of 8,628 WUS in the region.

Background

Cervical cancer is a significant health problem in Indonesia, with a high incidence rate and mortality rate. According to the Indonesian Ministry of Health, cervical cancer is the second most common cancer among women in Indonesia, accounting for 12.6% of all cancer cases. Early detection of cervical cancer is critical in reducing its risk and improving treatment outcomes. The IVA method is a simple and cost-effective method for early detection of cervical cancer, which involves a visual examination of the cervix using acetic acid to detect abnormalities.

Methodology

This study aims to analyze the implementation of the early detection program for cervical cancer with the IVA method at the Sei Lekop Puskesmas in 2018. This study uses a descriptive qualitative design and involves in-depth interviews with 9 informants, including the Head of Puskesmas, IVA officers, health cadres, patients, and non-patients. The results of this study provide insights into the implementation of the IVA program at the Sei Lekop Puskesmas and identify the challenges and obstacles faced by the program.

Results

The results of this study show that the implementation of the IVA program at the Sei Lekop Puskesmas has not been going well. One of the main obstacles is the role of health workers who have not been maximized in counseling activities at the community level and lack of cooperation with other sectors. In terms of funding, although funds are sourced from the Health Operational Agency (BOK) and the availability of sufficient materials and equipment, the Sei Lekop Puskesmas still faces constraints in the form of the absence of cryotherapy tools and special rooms for the implementation of IVA. In addition, officers responsible for the IVA program also have other duties and responsibilities that can reduce their focus on this early detection program.

Discussion

The results of this study highlight the importance of effective implementation of the IVA program at the Sei Lekop Puskesmas. The lack of cooperation with other sectors and the absence of cryotherapy tools and special rooms for the implementation of IVA are significant challenges that need to be addressed. In addition, the role of health workers in counseling activities at the community level is crucial in increasing awareness and participation in the IVA program.

Recommendations

Based on the results of this study, the following recommendations can be given to increase the effectiveness of the early detection program of cervical cancer at the Sei Lekop Health Center:

Increase education to cadres regarding the importance of IVA examination: Health workers need to be educated on the importance of IVA examination and its role in early detection of cervical cancer.
Socialization activities at the community level need to be improved: Socialization activities at the community level need to be improved to increase awareness and participation in the IVA program.
Strengthen cross-sectoral cooperation: Cross-sectoral cooperation needs to be strengthened to increase the effectiveness of the IVA program.
Carry out IVA examinations in an integrated posyandu: IVA examinations need to be carried out in an integrated posyandu to achieve more optimal program implementation.

Conclusion

In conclusion, the implementation of the early detection program for cervical cancer with the IVA method at the Sei Lekop Puskesmas has not been going well. The lack of cooperation with other sectors, the absence of cryotherapy tools and special rooms for the implementation of IVA, and the role of health workers in counseling activities at the community level are significant challenges that need to be addressed. Based on the results of this study, recommendations have been given to increase the effectiveness of the early detection program of cervical cancer at the Sei Lekop Health Center.

Limitations

This study has several limitations. Firstly, the study only focuses on the implementation of the IVA program at the Sei Lekop Puskesmas and does not provide a comprehensive overview of the implementation of the IVA program in all Puskesmas in Batam City. Secondly, the study only involves in-depth interviews with 9 informants, which may not be representative of the views of all stakeholders involved in the IVA program.
